We approach clinically severe obesity as a genetically based condition that is amplified by a variety of dietary, social, psychological, and environmental factors. Diet, medication, exercise, and counseling programs alone are ineffective for many patients.
Sleeve gastrectomy and gastric bypass surgeries reduce stomach volume, decrease hormones that stimulate the appetite, reduce insulin resistance, and increase pancreatic insulin secretion. Gastric bypass also causes food to bypass the duodenum and may further improve diabetic control.
Adjustable gastric band surgery was popular for many years because the band increased satiety and decreased the rate of food intake. However, weight loss was insufficient for many people, and many of the bands must be removed because of complications. Hence, we are no longer implanting bands. We are happy to work with patients who currently have a band and are making the best of it. We also see patients who wish to have the band removed and who may wish conversion to sleeve gastrectomy or gastric bypass.
Type 2 diabetes, essential hypertension, sleep apnea, and back and joint problems are just a few of the more than 65 major medical problems that are associated with obesity. Weight reduction can lead to major improvement or even resolution of many of these problems.
Thus gastric bypass and sleeve gastrectomy are tools, that when combined with behavior modification, psychological adjustment, and reasonable exercise, can result in significant and lasting weight reduction and improvement of obesity related conditions.
